Posting is usually done by computer more or less but coc!k-up is
usually by the admin staff. For example, they print out ur SAF100
(now dunno still got or not) then they forget take to Post Office
to mail. Happens all the time. Or FFI letter got stash into a pile
of documents then the admin staff forgets about it. Like all
computer system, the weakest link is ususally the human in the
end.
