You thank me, I thank the discovery channel. 
I watched this documentary about a plane crash where
they partially attributed the cause to it being an engineer used a
different although workable part for a the plane's maintainence,
and didn't register the change into the log book. The part in
questioned turned out to be screws, no bigger than your hand.
